Why Pre-register?

• Expedite the registration process in order to decrease patient wait times. We know your time is valuable.

 

Pre-registration

•Please give 24 hour notice for online pre-registration. If you are unable to meet this time frame, pre-registration can be completed by telephone prior to your appointment.

•You can call pre-registration at (574) 535-2472. Monday through Friday. 7 a.m. to 8 p.m. Please leave a detailed message and leave a phone number where you can be reached.

 

Pre-certification Process

•Your test may require pre-certification, please call your insurance company and verify before your date of service.

•Please make sure to call us with your authorization number, your doctor may be doing this service for you, please contact them.

•You may call 574-535-2400 to leave your pre-certification number.

•Per our service rendering policy, we will need to have your pre-certification number 24 hours before your scheduled test/procedure. If we do not receive the precertification your test may be rescheduled. This may prevent you from receiving the whole charge, depending on your insurance policy.

 

Payment Arrangements

If uninsured, please be prepared to pay a deposit at the time of service.
If you need financial assistance, please call the financial advocates at 574-535-2607.

 

What do I need to bring with me?

Please bring insurance cards.
If you have a copy of the order for your test, please bring that also.

Where do I go?

You will be checking in with registration located in the main lobby, unless instructed otherwise.
They will direct you to your point of service.
